Transition from pediatric to adult care in patients with type 1 diabetes. The current situation in Argentina

Objetive: to evaluate the impact of the transition process from pediatric to adult care in patients with type 1 diabetes, and to gather information for a diagnosis of the present health care situation regarding this matter in Argentina.
Material and methods: observational study where all the patients that had been followed-upat the Hospital Prof. Dr. Juan Pedro Garrahan and had been transferred to adult care from 2001 to 2006 were identified through hospital registries. Clinical
and biological information of metabolic controls corresponding to the last year of the pediatric follow-up, and the last year of the follow-up in the adult center were evaluated. Social and demographic data and information about adult health care were
assessed. Data were analyzed with the Stata 11 software.
Results: 38 patients were included, 17 male (45%), mean age 21,7 years (r: 15,6 y-26 y), mean age at diagnosis 10,93 ± 2,54 y (range: 5,5-14,9 y). Mean HbA1 in the last year at the pediatric unit was 8,8% ±1,9 vs. 9,9% ±3,2 at the adult unit (p:0,0048;
CI: 0,35-1,82, T test). Patients with previous poor control worsened during the transition process. Significant variables were assessed through logistic regression; it was found that the metabolic control during the last year at the pediatric unit, and having a treating physician were associated to metabolic outcome some years after the transition process.
Conclusions: the transition process worsened metabolic control of patients, mainly those with poorer control at the pediatric stage and without having a treating physician.
