Abstract

It is not news that the prevalence of diabetes mellitus (DM) worldwide continues to rise. And while this goes hand in hand with the rise in obesity and what we know as DM2, the prevalence of autoimmune DM (AMD) is also on the rise. Likewise, the form of presentation of AMD has different faces depending mainly on the age of onset.

Classically, AMD has been described in the infant and adolescent stage and is the most frequent form of presentation in this age group. It is characterized by a polygenic genetic predisposition where class II major histocompatibility complex (MHC II) polymorphisms are the main determinants of the disease. At this stage of life, autoimmunity is aggressive and generally causes abrupt destruction of the beta cell mass, rapid drop in C-peptide, and symptomatic hyperglycemia that, if left untreated, rapidly leads to diabetic ketoacidosis.
