Patient selection for surgery

Adequate patient selection for bariatric and metabolic surgery is crucial, specially for patients diagnosed with grade 1 obesity. Until now, candidates’ selection focuses on BMI, despite a majority of the evidence suggesting basal BMI as a non-predictive factor of T2D remission and T2D improvement not depending on weight loss on an exclusive basis. 

BMI-based current criteria should be replaced by values related to the metabolic condition of each patient. Various predictive scoring systems would help in patient selection process as regards the patients who might likely succeed in the surgical treatment and in having realistic expectations about surgical results. Nonetheless, only a handful of currently available scoring systems match.
